DOT Drug Test: What Every Driver Should Know and How It Can Affect Your CDL

If you are a CDL-licensed driver, you should know that the DOT drug test detects five main groups of substances that can immediately take you off the road:
✅ Marijuana (THC)
✅ Cocaine
✅ Amphetamines (Adderall, Vyvanse, Methamphetamine, MDMA, MDA)
✅ Opioids (Codeine, Morphine, Heroin, Hydrocodone, Oxycodone, Oxymorphone, Hydromorphone, Fentanyl, Methadone)
✅ PCP
Any positive result constitutes a violation, is reported to the FMCSA Clearinghouse, and prohibits you from driving until the DOT Return-to-Duty process is completed.
Other substances that may cause a positive result or disqualify you:
Pain medications: Tramadol, Demerol, Dilaudid
Anxiety medications: Xanax, Valium, Klonopin (may result in “Unfit” on the DOT physical exam)
Sleep aids: Ambien, Lunesta
ADHD medications: Adderall, Vyvanse (high risk if proper medical documentation is missing)
Weight loss pills: many contain amphetamine derivatives
CBD products: often contaminated with THC
Marijuana edibles: gummies, brownies, vapes → even a single gummy can trigger a positive test
Coca tea and imported herbs: high risk of cocaine-like metabolites
Even with a prescription, many opioids and stimulants can result in a “Negative with Safety Concern” outcome, which also prohibits driving until formally cleared by the MRO and your physician.
A single pill, gummy, or tea can put your CDL at immediate risk. Always review your medications with your doctor and your employer before taking any substance.
